Student Nomination Form: Accelerate 

Welcome to the Boston Partners in Education student nomination form for our In-Person Accelerate program offering. Accelerate is designed to support students in Kindergarten through 8th grade, in one-on-one or small group (2-4 students) matches. 

Requesting In-Person Accelerate Support

If you are looking to nominate high school students or whole classrooms, please review our program offerings here and decide which is best for your students.

Terms & Conditions
To begin, please verify that you agree to the following terms and conditions:
  1. I will provide the mentor with a designated space and materials. This can be in a classroom with one specific set of students, or in a shared space with programming occurring at different times.
  2. I will communicate with the Academic Mentor(s) to discuss student progress and provide feedback.
  3. I will fill out a survey from Boston Partners in Education two times per year regarding student progress.
  4. I will assist Boston Partners in Education in collecting parent/family permission slips by sending them home with students and submitting them to Boston Partners in Education staff.
  5. I will respond to periodic email check-ins from Boston Partners in Education staff.
  6. I will update Boston Partners in Education staff if the students served by the match change.
  7. I will communicate with the Academic Mentor about their student(s) attendance to reschedule as needed.
