Student Nomination Form: Aim High

Aim High is designed to support students in 9th through 12th grade, in one-on-one or small group (2-4 students) matches. 
If you are looking to nominate students in grades K-8 or whole classrooms, please review our program offerings here and decide which is best for your students.
Requirements for 2020-2021
For School Year 2020-2021, the following is required to match students in Aim High:
  • Use of Zoom breakout rooms — Google Meet does not currently have this capability
  • Student participants must have a reliable internet connection and working audio/video capability
  • Mentoring sessions must be on a consistent schedule — at the same time, on the same day, once a week

Terms & Conditions

Please verify that you agree to the following terms and conditions:
  1. I will provide the necessary electronic resources for mentors to work with their students, including videoconferencing links and curriculum materials as appropriate. 
  2. I will communicate with academic mentor(s) to discuss student progress and provide feedback.
  3. I will communicate with my student about receiving an academic mentor and the purpose of Boston Partners in Education prior to the match starting.
  4. I will fill out a survey from Boston Partners in Education up to two times per year regarding student progress.
  5. I will assist Boston Partners in Education in collecting parent permissions by emailing Boston Partners’ permission form link to the parents of the students I nominate.
  6. I will respond to periodic email check-ins from Boston Partners in Education staff.
  7. I will update Boston Partners in Education staff if the students served by the match change.
  8. This request has been approved by my principal/headmaster.
  9. I understand that all Boston Partners matches for SY 2020-2021 will be online, regardless of whether the district approves a return to in-person mentoring.
  10. I understand that in SY 2020-2021, Aim High matches require the use of Zoom breakout rooms and must be appropriately monitored by myself or another school employee.
